REACHING THE MOST VULNERABLE- ONE HOUSEHOLD AT A TIME
How Nurtured to Thrive Uganda's community outreach programme is transforming early childhood health across Jinja City's most underserved neighborhoods.
In the bustling lanes of Jinja City, where the Nile marks the horizon and everyday life moves at the rhythm of tight community bonds, there are children and young women whose potential is quietly at risk, not from lack of strength or ability, but from gaps in support, information, and care. Nurtured to Thrive Uganda (NTT) exists to bridge those gaps.
Through its flagship Nurturing Futures Plus (NF+) programme, NTT has built a structured community outreach model that goes beyond clinic walls, meeting families where they are, building trust where it matters, and delivering integrated care that no single institution could provide alone.
"We don't wait for families to find us. We go to them, with compassion, with knowledge, and with the conviction that every child deserves a thriving start."- Lubega John Paul, Founding Director of Nurtured to Thrive Uganda
What the outreach programme looks like
The NF+ outreach programme operates through a coordinated network of community health workers, social workers, and programme officers who conduct structured home and community visits across Jinja City. Each visit is purposeful, guided by standardized protocols, documented with programme-specific tools, and anchored in a wholistic care philosophy that attends to the child and the family together.

Stories from the community
Behind every visit record is a human story.
Outreach work is slow, relational, and often invisible, but its effects ripple
outward across households and generations.
“Before
the NTT team started visiting us, I didn’t know what to look for in my child’s
development. Now I keep a record. I know when he reaches a milestone. I feel
like I am part of his progress.”
- Caregiver,
Walukuba Division, Jinja City
“The
follow-up after discharge made all the difference. We didn’t feel abandoned.
The community health worker came back, checked on us, and helped us connect to
the clinic.”
- Mother of a
post-discharge child, NF+ cohort
THE
APPROACH
Integrated care as a philosophy
What distinguishes NTT’s outreach from
conventional community health programmes is its commitment to integration. The
NF+ Integrated Care & Empowerment Model recognizes that a child’s health is
inseparable from their caregiver’s stability, their family’s economic security,
and the community’s capacity for collective care. Outreach workers are not just
health messengers, they are connectors, advocates, and trusted allies.
Every visit follows a structured protocol,
with post-discharge follow-up procedures (NTT-NF+-PROT-001) ensuring that no
child falls through the cracks after leaving a health facility. Standardized
forms track everything from initial assessments to community referrals,
creating a documented trail of care that informs programme improvement and
accountability.
LOOKING
AHEAD
What comes next
Nurtured to Thrive Uganda is currently
deepening its research foundation, with an ongoing prospective cohort study on
early childhood development in the Busoga Sub-Region. The evidence gathered
through outreach will directly inform the next evolution of the NF+ programme, ensuring that community-level learning shapes policy and practice, not the
other way around.
The goal is not scale for its own sake, but
depth, a model so well-rooted in community trust and evidence that it becomes
a replicable template for child-centered outreach across Uganda and beyond.
